HSP-HC is committed to serving the Guam community with a high quality of personal health care services in the home. Our professional and para-professional staff are screened and carefully referred to provide services with a personal touch. Our staffs of Registered Nurses, Licensed Practical Nurses, Home Health Aides, Physical Therapist, Occupational Therapist, Medical Social Worker, Registered Dietitian, Respiratory Therapist are involved in the plan of care.

 

At HSP-HC the patient is our paramount concern. It is our constant focus to keep our health care team informed and updated on the latest technology and procedures by providing frequent in-service education. Our highly skilled, multilingual staffs are duly qualified professionals. We offer seven (7) days per week, twenty four (24) hours on-call service with an active list of employees.

 

Creating a warm, caring environment is an essential ingredient in the expedient recovery of our patients. Our staff understands an important facet of the agency - philosophy is to teach another caregiver or the patient to perform the tasks that agency staff is performing so that the agency can decrease and eventually cease service. We are committed to preserving the patient in the home and including the entire family in the patient's therapy by way of educating family members in the nature of the patient's illness and specific needs.

1. Skilled Nursing Services:

Registered Nurses along with a team of Licenses Practical Nurses and Certified Nurses Aides provide skilled nursing care. Nursing care in the home setting can be long-term IV therapy for treatment of infections, hydration or nutrition management; medication management and monitoring; wound care management, respiratory monitoring and management are some examples of skilled nursing care.


2. Rehabilitation Services:

A team of physical therapist, occupational therapist and speech therapist provide restorative and rehabilitation services in the home setting in efforts to improve functional and mobility status of individuals that have limited mobility yet show a good potential of improvement.


3. Other ancillary services:

To address all issues faced in health care, HSP-DC provides ancillary support to the nursing and rehabilitation team. These services include Medical Social Worker, Registered Dietitian, and Respiratory Therapist.


4. Disease Management Programs:

Disease management uses interdisciplinary approach of the clinical team on common chronic diseases on Guam. The current programs offered in home care are diabetes management, chronic respiratory disease management and heart failure management. Each program includes self-care management techniques, patient education, and provider training. Disease management provides individualized care plans based on clinical guidelines to manage individuals with treatable chronic diseases.


5. Patient Education:

All individuals in home care receive continuous patient education by the entire clinical team.
