How to begin HSP services

1. Physicians MUST refer to HSP for home care services.
2. HSP coordinates physician orders
3. HSP Healthcare professionals deliver the care.
4. Hospice benefits are available to Medicare Part A and Other Insurance beneficiaries with proper certification.

Questions to consider when looking for a home care agency:

1. How long has the agency been serving the community?
2. Is the agency an approved Medicare provider?
3. Does the agency have a current license to provide health services?
4. Does the agency offer a “Patient’s Bill of Rights” that describes the rights and responsibilities of both the agency and the client being cared for?
5. Is the quality of care certified by a national accrediting body such as the Joint Commission (JC)
